1. For all representatives of US Government and US Agencies. Who is traveling to Kazakhstan on official purposes, an official letter from the Special Issuance Agency, Passport Services of the United States Department of State has to be provided.

2. For all representatives of United Nations, an official letter from Protocol and Liaison Service to the United Nations.

3. A written statement with a request to issue a visa, including a trip purpose

4. A completed visa application form. (PDF document, Word document)

5. A passport size photo (color or black-white) of the applicant.

6. Your original and valid passport, with at least one blank page for a visa. Your passport must be valid for at least six months longer than the terms of the requested visa. For example if requesting a one month visa, your passport must not expire for at least seven months. Also a photocopy of the first page of passport.

7. A money order or company check made payable to the Consulate of Kazakhstan in New York is accepted (no cash or personal checks accepted).

8. In order to receive your passport back please provide with the United States Postal Service (USPS) envelope only. Please note that the envelope must be pre-paid and self-addressed. For your convenience it is recommended to keep a tracking number of the return package so you can check the delivery status with USPS after five working days since the arrival of your request to the Consulate General. The Consulate will not accept any other type of mail (Do not send FedEx, UPS, DHL and other envelopes).
